Comparação dos critérios de classificação da obesidade infantil e sua associação com a asma

Resumen
Objetivos: verificar a prevalência de asma e obesidade em escolares e adolescentes na cidade de Juiz de Fora - Minas Gerais, estudar quatro diferentes parâmetros antropométricos para definição da obesidade infantil e identificar a diferença entre os mesmos quanto à associação com a asma e a asma grave. Métodos: estudo observacional do tipo transversal. Utilizou-se do questionário ISAAC e dados antropométricos. O trabalho foi realizado com escolares (6 a 8 anos) e adolescentes (13 a 15 anos). Nas escolas, selecionadas aleatoriamente, eram realizadas as medidas antropométricas. As comparações foram realizadas a partir do teste t-student ou Mann-Whitney. A concordância da classificação da obesidade entre os diferentes parâmetros estudados foi analisada através do coeficiente Kappa. Resultados: 394 estudantes 58,1% tinham entre 6 a 8 anos e 54,3% eram do sexo feminino. A prevalência de asma ativa nos escolares foi de 18,6 % e de 12,7% nos adolescentes. A prevalência da obesidade foi de 24% e de 28% respectivamente. Houve boa concordância da circunferência abdominal (coeficiente Kappa 0,68) e da razão cintura-altura (coeficiente Kappa 0,71) em relação ao escore Z IMC na comparação entre os parâmetros antropométricos de classificação da obesidade. Observou-se associação da relação cintura-altura (p = 0,044) e do escore Z IMC (p= 0,017) com a asma e da circunferência do pescoço com a asma grave ( p= 0,004). Conclusão: A relação cintura-altura e a medida da circunferência do pescoço são importantes medidas antropométricas na definição da obesidade infantil associada à asma, sendo medidas de fácil execução na prática clínica.
